Preparation

The following page provides an overview of the preparation of patients for diagnostic or interventional examinations at our institute. In order to ensure a smooth examination procedure.

In addition, it should be helpful to take precautions for patients with KM allergies, renal insufficiency and implants for the examinations.

The following laboratory values are required before administration of contrast medium:

  • Serum creatinine
  • Glomerular filtration rate
  • TSH

These should not be older than 14 days. If pathological, current values. At TSH if pathologically current values including T3 und T4.

Before interventional procedures
  • Serum creatinine
  • TSH
  • Quick, PTT und Thrombozyten and Hb

These should not be older than 14 days. If pathological, current values. At TSH if pathologically current values including T3 und T4.

  • Food restriction is not necessary before a KM-assisted examination.
  • In case of increased risk, contrast medium allergy or a puncture, a food abstinence of 6 hour n is required.

Special preparation of patients

Experimental studies on the transplacental passage from the contrast midline could not detect contrast media in fetal plasma or amniotic fluid. In animal studies, no evidence of a mutagenic or teratogenic effect of iodine-containing or gadolinium-containing contrast media could be observed.

If a radiographic or MRI examination is required, contrast media containing iodine or gadolinium may be given.

Contrary to the recommendation in the package inserts of the contrast medium manufacturers, the 24-hour breastfeeding break according to the guidelines of the European Society of Urogenital Radiolgy (ESUR) does not appear to be necessary. Breastfeeding can be continued normally after administration of contrast media containing iodine or gadolinium.

  • The oral antidiabetic agent meformin is widely used and is primarily renally eluted. However, transient renal function impairment due to iodinated contrast agent increases the risk of metabolic acidosis.
  • In normal renal function: discontinue metformin for 48 h after contrast administration.
  • If renal function is impaired, metformin should be discontinued 48 h before and 48 h after contrast administration.

Preparation of patients before a CT and MRI examination
 

Laboratory values

  • Serum creatinine
  • Glomerular filtration rate
  • TSH

These should not be older than 14 days. If pathological, current values. At TSH if pathologically current values including T3 and T4.

Metformin intake
  • In normal renal function: discontinue metformin for 48 h after contrast administration.
  • If renal function is impaired, metformin should be discontinued 48 h before and 48 h after contrast administration.

Food and fluid abstinence
None required; only in case of known allergy to contrast medium, known tendency to aspiration.

Known contrast agent allergy
Evaluation of other imaging with the referring physician or premedication of the patient.
Hydration
sufficient supply of fluids must be ensured before and after the CT examination. (approx. 1.5 l)

After administration of Buscopan or administration of H1/H2 inhibitors for KM allergy.
Do not drive or operate machinery 24h after the examination.

Patients with metallic implants
As in the case of pacemakers, defibrillators, or ear implants, they may not be examined in MRI, or only after the strictest indications have been established. In the case of heart valves, there is only a contraindication for older models. Patients with joint prostheses can generally be examined in MRI.

Food and fluid abstinence
None required; only in case of known allergy to contrast medium, known tendency to aspiration.

Known contrast agent allergy
Evaluation of other imaging with the referring physician or premedication of the patient.

Hydration
sufficient supply of fluids must be ensured before and after the MRI examination. (approx. 1.5 l)

After administration of Buscopan or administration of H1/H2 inhibitors for KM allergy.
Do not drive or operate machinery 24h after the examination.

2 Tage vorher: keine blähende Speisen, Brot Hülsenfrüchte, fett-reiche Kost, Milch, Gemüse, Obst, Salat, Vollkornprodukte, Reis, Fleisch, Fruchtsäfte, kohlensäurehaltige Getränke und Schokolade.

1 Tage vorher:Nur noch flüssige Nahrung (z.B. Suppe) einnehmen und über den Tag verteilt 3 Liter kohlensäurefreies Mineralwasser trinken.

Um ca. 18 Uhr: die PREPACOL-Lösung mit einem halben Glas Wasser einnehmen und ein großes Glas Wasser (1/4 Liter ) nachtrinken.
Danach bis zur Untersuchung nicht mehr essen!

Um ca. 22 Uhr: die 4 PREPACOL-Tabletten unzerkaut und wieder mit einem großen Glas Wasser einnehmen.
Bitte so lange Wasser trinken, bis Sie nur noch glasklare Flüssigkeit ausscheiden.

Untersuchungstag:Am Morgen noch erlaubt: Tee oder Kaffee, aber ohne Milch. Keine feste Nahrung, jedoch reichlich kohlensäurefreies Mineralwasser!

Nach Gabe von Buscopan oder Gabe von H1/H2-Hemmern bei KM-Allergie
24h nach der Untersuchung kein Autofahren und keine Maschinen betätigen.

Preparation of interventional procedures

Laboratory values

  • Serum creatinine
  • Urea
  • Glomerular filtration rate
  • TSH

These should not be older than 14 days. If pathological, current values. At TSH if pathologically current values including T3 and T4.

Metformin
  • n normal renal function: discontinue metformin for 48 h after contrast administration.
  • If renal function is impaired, metformin should be discontinued 48 h before and 48 h after contrast administration.

Food and fluid abstinence
None required; only in case of known allergy to contrast medium, known tendency to aspiration.

Antiplatelet agents and anticoagulation
e.g. Aspirin, ASS, Glopridogrelusw. e.g. Marcumar must be discontinued 5 days prior to an intervention

Food and fluid abstinence
Fasting required for 6 hours

Hydration
sufficient supply of fluids must be ensured before and after the CT examination. (approx. 1.5 l)

After administration of Buscopan or administration of H1/H2 inhibitors for KM allergy.
Do not drive or operate machinery 24h after the examination.
